Doctors: Dutch prince may never wake

(CBS/AP) Dutch Prince Johan Friso, who was buried by an avalanche while skiing off-trail in Austria last week, has suffered massive brain damage and may never regain consciousness, his doctors said Friday.

Dr. Wolfgang Koller, head of trauma at the Innsbruck hospital where Friso is being treated, told a news conference broadcast live on Dutch national television that it took nearly 50 minutes to reanimate the prince after he was pulled from the snow. He had been buried for 25 minutes before rescuers found him.

"It is clear that the oxygen starvation has caused massive brain damage to the patient," Koller said. "At the moment, it cannot be predicted if he will ever regain consciousness."

Friso will be moved at a later date to a rehabilitation clinic for further treatment. But Koller cautioned that it may take years before he awakens from his coma, if he ever does, and any recovery from such significant brain damage is a process of "months or even years."

The accident occurred as Friso was skiing off-piste in Lech, Austria, despite avalanche warnings, with a childhood friend from the alpine village that the Dutch royal family has been visiting each winter for years.

The friend was carrying an avalanche "air bag" and escaped without serious injury. Friso was found with the help of a signaling device he was carrying and was flown by helicopter to the Innsbruck Clinic.

But "50 minutes of reanimation is extremely long. You could say too long," Koller said.

The doctor said that due to protocols for minimizing brain damage after such an accident, it had only been possible to conduct an MRI scan of Friso's brain on Thursday.

Frisco, 43, is the second of Dutch Queen Beatrix's three sons. He lived in London with his wife, Princess Mabel, and their two daughters, Luana, 6, and 5-year-old Zaria.

Members of the family, including Queen Beatrix, Friso's older brother Crown Prince Willem-Alexander, and others have traveled to and from the hospital in a steady stream amid a grim atmosphere in the week following the accident.

WEDNESDAY, Feb. 22 (HealthDay News) -- News coverage of alcohol's role in violent crime and fatal accidents may persuade the public to give stronger support to alcohol-control laws, new research suggests.

It is estimated that drinking is involved in nearly one-third of deaths from accidents and violent crime. Most news reports of such cases, however, make no mention of alcohol, according to the authors of the study, published in the March issue of the Journal of Studies on Alcohol and Drugs.

"People have some awareness of the social cost that alcohol can have," study author Michael D. Slater, of Ohio State University, in Columbus, said in a journal news release. "But only a small fraction of news stories on violent crime and nonmotor-vehicle accidents acknowledge the contributing role of alcohol."

This means that many people don't realize how often alcohol plays a role in violence and accidents that don't occur on the roads. This lack of awareness may dampen public support for alcohol-control laws such as strict enforcement of underage drinking rules or bans on serving alcohol to drunk customers, Slater said.

In the study, Slater and his colleagues surveyed 789 adults and found that they were more likely to support alcohol-control laws after they read media stories about violent crime, vehicle crashes and other accidents involving alcohol.

"I think this buttresses the idea that media coverage does matter," Slater said. "Alcohol, as a public-health issue, is not as front and center as it might be if there were more news coverage."

In areas that already have alcohol-control laws, public support is still important because local resources are needed to enforce those laws, he noted. Local authorities should mention any role of alcohol when giving details about crimes and accidents to the media, Slater added.

If someone had slipped Lady Macbeth some hand sanitizer, she might have been a much happier person. At least that?s the suggestion of a new study that found that washing your body can help you ?wash away your sins.? Literally. People who physically wash themselves wipe the slate clean of guilt, fear and even doubts about past decisions. Next time you tell a lie, you may want to wash your own mouth out with soap to avoid the guilt.

University of Michigan researchers Spike W. S. Lee and Norbert Schwarz looked at dozens of studies that linked the idea of ?moral cleanliness? with physical cleanliness. In one study, for example, people who were asked to recall an immoral act they?d committed felt less guilty about it after they were given an antiseptic wipe to use. In another, people told to tell a lie wanted either mouthwash or hand sanitizer afterward, depending on whether they had told or typed the lie. (Apparently, we need to cleanse the appropriate body part or it doesn?t work.)

So what sent researchers down this particular path of inquiry? ?Many psychologists will tell you it has something to do with my mom,? laughed Lee, a doctoral student. ?It?s not as crazy as it sounds.?

You see, Lee?s mother in Hong Kong is a clean freak. ?If you get more than 2 feet into her apartment without taking off your shoes, she?s ?No, no, no!?? he says.

Lee thought of her when he read a 2006 scientific paper that describes the psychological link between physical and moral cleansing, a nearly universal element of religious ceremonies for thousands of years. Researchers refer to this as the ?Macbeth effect,? named after the aforementioned Lady Macbeth of Shakespeare fame, who tried to assuage her guilt at the wash basin (?Out, damned spot!?) after encouraging her husband to murder King Duncan. No, Lee?s mother hasn?t done away with any royalty. But thanks to her, he did become intrigued with how we think about cleanliness and the ways in which that may have had some evolutionary advantages. ?Think about what elicits the strongest disgust reactions in humans -- bodily fluids, feces, which are also harmful to us,? he explains. ?If you touch them, you definitely want to purify your body so you wouldn?t be contaminated.? Making the metaphorical leap to include what we see as unclean acts came later.

Lee admits his main interest is in how humans think -- and behave -- metaphorically. And the metaphors we use aren?t limited to cleansing. ?When we say ?He is a very warm person,? we don?t mean he has a high body temperature. We mean he?s psychologically social, cheerful and generous. When we say ?We?re pretty close friends,? we?re not talking about physical distance. We use the word ?seeing? as in ?I see what you mean? to mean understanding. On the average, people unconsciously use four metaphoric expressions a minute.?

In other words, studying the psychological effects of cleansing is a way to ?see? how humans think and why they behave the way they do (so appropriate in an election year).

And our cleansing metaphor, rather than just giving us a physical way to get rid of guilt, helps us become better people, says Lee. ?Cleansing plays an important role across all religions and cultures. It signals a new phase of life, like a reborn Christian, which gives you a new moral self-image to live up to.?

So if you?re feeling bad about something and want a clean slate, you may want to start with a shower.

In an analysis (.pdf) published Thursday in the online edition of the journal Science, a team led by virologist Peter Palese of Mount Sinai School of Medicine in New York argue the mortality rate from H5N1 bird flu is under one percent, much lower than the WHO's estimated fatality rate of 59 percent, Reuters reports. And in a paper published Friday in mBio, the journal of the American Society for Microbiology, Michael Osterholm of the University of Minnesota and a member of the U.S. National Science Advisory Board for Biosecurity (NSABB) and a colleague "conclude that H5N1 kills up to 80 percent of people it infects," a rate higher than WHO estimates, according to the news agency.

The studies "ad[d] fuel to the heated controversy over publication of bird flu research" in the journals Science and Nature describing how two teams created H5N1 strains that are easily transmissible among ferrets, which are used as lab models for humans, Reuters writes (Begley, 2/23). Fears that terrorists possibly could use the information prompted the U.S. National Science Advisory Board for Biosecurity in December to request the scientists redact some information prior to publishing their study results and investigators in January to institute a 60-day moratorium on bird flu research, USA Today's "Your Life" notes. "A WHO summit that ended this week called for full publication of the two studies and for an extended halt to such research until stronger safety measures were assured in labs," the blog writes (Vergano, 2/23).

Rurals Prove Size Is No Barrier to Reaping Lean Rewards

North Carolina collaborative creates a model for small hospitals everywhere.

Admit it. When you think about who's pioneering groundbreaking business techniques, rural hospitals don't come to mind first. But that's happening in western North Carolina, where five rural hospitals got together in 2008 to figure out how to do what they do better, from both a quality and a fiscal perspective.

Like many business executives today, these hospital leaders suspected that Lean methodology could help them step up their games. But because their operations are small, with tight margins and limited staffs, investing money and time on Lean training was something none could easily afford alone. So, in 2008, they formed the North Carolina Rural Hospital Lean Culture Transformation Collaborative.

"By doing it together they could learn and implement Lean more efficiently," says Jeff Spade who, as vice president of the North Carolina Hospital Association and executive director of the North Carolina Center for Rural Health, spearheaded the collaboration. For instance, because collaborative members are geographically close, they can share a Lean coach from improvement consultant Simpler Healthcare. The coach comes to the area once a month and moves from one hospital to the other. That saves 30 to 40 percent of the coaching expense, Spade says.

The first step in forming the collaborative was a series of informal workshops to create dialogue among the hospital CEOs. One-on-one sessions with executives nurtured a commitment to Lean and to the goal of improving care, eliminating inefficiencies in various processes and containing expenses. A "funding quilt" stitched together grant support from the Duke Endowment and a variety of federal, state and other sources.

The five hospitals agreed to a three-year collaborative to establish a base for Lean culture transformation. Each of the five hospitals hired at least one Lean coordinator, mainly from their respective current workforces. "We believe that person is usually already employed in the hospital," Spade says. Coordinators have come from nursing, infection control and quality improvement, though some do have Lean experience in industries other than health care.

A three-week training program was conducted for the designated Lean coordinators and the hospitals' executives. After that, the real work began.

A strategic analysis was conducted to refine how hospital leaders perceive their operation now and the future improvements to be accomplished. Then each hospital selected so-called, rapid-improvement events. The RIE process pinpoints a specific operational area ? say, revenue cycle management. It spells out what problems need to be addressed and why, the steps for solving them, and how success will be measured. Everyone involved in the particular activity or process participates.

The overarching goal is to improve quickly and then continually improve "in small pieces over time," Spade says.

Each hospital in the collaborative chose different RIEs. For example, McDowell Hospital focused on the emergency department and surgery. Caldwell Memorial Hospital chose revenue cycle, inpatient flow, surgery, the physician network, the pharmacy and the safety net.

The results amazed everyone. Most RIEs met or surpassed the pre-established goal of 50 percent improvement within three years, and some hospitals have seen an overall return on their investment of greater than 6-to-1. Among the results were:

• A 60 percent reduction in transportation time of inpatients to and from imaging at Ashe Memorial Hospital.
  
• A 35 percent reduction in turnaround time for the top five laboratory tests ordered; and a reduction from 60 minutes to 28 minutes from the time the patient presents in the emergency department to the first intervention at Cannon Memorial Hospital.
• A reduction in patient room turnaround time at inpatient discharge from an average of 90 minutes to 45 minutes. That brought an additional benefit: Aggregate capacity improved by 230 patient days, resulting in $398,750 net revenue with no additional staff or rooms at Caldwell Memorial Hospital.
  
• An improvement of more than $400,000 in the revenue cycle at Blue Ridge Memorial Hospital.

"Caldwell Memorial Hospital is a leader among Lean-managed hospitals in the United States," Spade says. Its three-year return on investment for Lean transformation is 6.25-to-1. It has conducted 77 Lean improvement events since September 2008 and now manages five value streams with five Lean coordinators.

With the success of the Lean initiative, the collaborative is reaching out to other health care organizations in the hospitals' communities, such as physician practices, public health departments and home health agencies. "Health care is a product of these organizations working together," Spade says. "Think Lean across organizations, not just in a single organization." That's going to be especially crucial as accountable care and value-based payment gain traction.

The initiative has been so effective that a similar collaboration was launched among rural hospitals in eastern North Carolina. In 18 months, the hospitals have achieved 37 Lean improvement events and engaged 285 associates and leaders in rapid-cycle management. The early ROI for the collaborative is 3.7-to-1.

The North Carolina model has been adopted in Nebraska and Oregon, and versions of it are spreading to other states.

Probe into gender abortion claims

Health officials have been asked to carry out an investigation "as a matter of urgency"

The Department of Health has launched an inquiry into claims that doctors agreed to carry out abortions on the grounds of the sex of unborn babies.

The Daily Telegraph said it had secretly filmed doctors at a number of British abortion clinics.

The doctors are said to have agreed to terminate foetuses when women did not want their baby because of its gender.

Health Secretary Andrew Lansley said sex selection was "illegal and morally wrong" and he had ordered an inquiry.

He said: "I'm extremely concerned to hear about these allegations. I've asked my officials to investigate this as a matter of urgency."

In its report, the Telegraph said doctors had admitted they were prepared to falsify paperwork so the illegal procedures could go ahead.

The newspaper said undercover reporters had accompanied pregnant women to nine clinics in different parts of the country.

In three cases, the Telegraph reported, doctors were recorded offering to arrange terminations after being told the women did not want to continue with the pregnancy because of the gender of the unborn child.

In England, Wales and Scotland abortions are allowed on certain grounds before 24 weeks of pregnancy including that:

• Continuing with the pregnancy would be a greater risk to the woman's life, physical or mental health than ending the pregnancy
• Continuing would be more of a risk to the physical or mental health of any of the woman's existing children
• There is a real risk the unborn child would have a serious physical or mental disability

Two doctors have to agree to the abortion, or one, in the case of an emergency.

Conditions are stricter for abortions carried out after 24 weeks. In Northern Ireland abortions are illegal usually, unless the mother's life is at risk.

Train GPs to spot smokeless tobacco use, says NICE

Draft NHS guidance published by the institute aims to tackle use of products such as gutka, pan masala, shupari, and betel quid among South Asian communities in England.

NICE wants health professionals including GPs, dentists, health visitors and midwives to undergo formal training to increase awareness of symptoms. This would help ensure GPs can educate their patients on the potential health risks and offer cessation advice.

Professor Mike Kelly, director of the NICE Centre for Public Health Excellence, said: ?Generally, awareness of the problems caused by this particular type of smokeless tobacco use is low within mainstream NHS services. As a result, there?s a clear requirement to assess the needs of local communities and provide targeted support to help people of South Asian origin stop using these products.?

Smokeless tobacco products are associated with serious health problems, such as oral cancer, serious tooth decay, MI, stroke and problems in pregnancy.

The use of these products is thought to be one of the main reasons why South Asian women are nearly four times more likely to develop oral cancers than women from other ethnic groups in England.

The guidance also calls for the NHS to work with South Asian communities to understand their concerns and create schemes to help people quit.

A consultation on the plans will run until 25 April.
